As a busy healthcare professional, we know how difficult it can be to find trusted community services to refer people in to.  At Simply Connect we only work with voluntary and community groups that welcome referrals from across Health and social care.

Feeling like you don't know where to turn?

Sometimes its difficult to know where to turn, sometimes you feel you can't speak to family members, friends or your GP about what your going through.  


Simply Connect is a safe space to find confidential support services, whatever issue you are struggling with.

What is social prescribing?

This is when you are referred or signposted into non-clinical services by a professional as a way of supporting or improving your health and wellbeing.

Social Prescribing Link Workers are new posts, funded by the NHS, who will support you and make referrals on your behalf into community services.
